  • I understand that I must make a one-time donation of at least $25 per Pet to Piggie Poo Rescue upon relinquishment of Pet.
  • Supply donations are also welcome in addition to monetary donations, but are not accepted in lieu of monetary donations.
  • I understand that the Pet may be transferred to another rescue organization, as is in the best interests of finding a qualified home for Pet.
  • I understand that a physical evaluation and temperament test will be performed by Piggie Poo to ensure adoptability before we accept Pet into our program. Piggie Poo reserves the right to refuse acceptance of any Pet and to require additional monetary donations to offset any veterinary costs or extended foster care for Pet.
  • I certify that I am the legal owner of Pet. I acknowledge that I am irreversibly relinquishing to Arizona’s Piggie Poo Rescue, Inc. all right, title, and ownership that I may have had with respect to Pet. Once I have given up Pet, I will in no way be entitled to have him/her back.
  • I understand that all attempts will be made to place Pet into a qualified home. While in the care of Piggie Poo, Pet will never be euthanized, except under the direction of a Veterinarian for the best interests of Pet. Piggie Poo Rescue will solely decide what action will be taken with Pet, to Pet’s best interest.
  • I give Piggie Poo permission to contact me at a reasonable time to make further inquiries about the Pet, as needed while in foster care.
