Animal Friends Furever , Inc. Adoption Application
Please complete the following adoption application. (You must be at least 21 year’s of age.) Please be sure to fill out this application completely, if the application is not filled out completely we will disregard it. We review applications quickly. We value your opinions and honesty, which will allow Animal Friends Furever Inc. to match owners and animals. This will ensure our goal of finding compatible homes for our rescue animals. Please remember that all of the staff at AFF are volunteers. Please be patient during the adoption process. Below are the steps involved. 1. Adoption Process Steps: 1) After we receive your application it will be reviewed by our board. If any additional questions need to be asked, they will either be e-mailed to you or you will receive a phone call from a member of AFF, 2) Vet and personal references will be checked. If your application is approved thus far, a member of AFF will call you to set up a house check date, 3) After you have been approved you will be able to talk to the foster of the animal 4) The last part of the adoption process is a member(s) of AFF collecting the adoption fee and your signing the Adoption Contract in your home in which you agree to certain conditions (Keeping the animal(s) as Indoor house pet(s), maintaining regular veterinary care, allowing an AFF member(s) to visit your home after placement of the animal(s), surrendering the animal back to AFF if you violate the contract. 61. Do you understand that you will be responsible for the neutering /spaying of this rescue animal, if he or she is not already neutered /spayed? A Vet certificate will be required after the surgery.

63. Do you understand and agree that there will be an adoption fee of at least $150.00 for a dog or at least a $100.00 fee for cats? NOTE: We do not accept personal checks. We will only accept cach or money orders for adoption fee's.
